Providers work with patients to address health areas of concern before they escalate, and also provide ongoing monitoring and personalized treatment plans to improve patients’ quality of life.

How to Obtain a Referral or Prescription Renewal
Patients can request referrals or prescription renewals through the MyFairHill electronic health chart by visiting our office or by calling the office at 215-324-0600.
A Referral Request Form must be filled out for all referrals. Please allow the office 24-48 hours (1-2 days) before pick up.
Prescription renewals will be refilled the same day unless the provider determines that an office visit is needed.
Sliding Fee Discount Program
At Fair Hill Community Physicians, no patient will ever be denied access to care based on their inability to pay for our services. With the Sliding Fee Discount Program, the amount you pay is based on your household size and income. This program not only allows us to provide vital healthcare services to more people in the community, but it puts less of a financial stress on all those who rely on our office for their care.
